摘要:

肺移植围手术期会发生不可避免的移植物缺血再灌注损伤(ischemia reperfusion injury, IRI),这严重影响患者的整体预后。如何防治肺移植术后的IRI仍然是世界性难题。目前已经有大量研究发现吸入一氧化氮(inhaled nitric oxide,iNO)可以治疗肺移植术后的低氧血症性呼吸衰竭和肺动脉高压,对防治IRI有巨大潜力。本综述总结iNO防治肺移植术后IRI的作用机制,针对使用iNO最佳时机、浓度、持续时间展开论述,并进一步分析其具体应用条件下的疗效、安全性及产生的毒副作用为临床提供指导。

Abstract:

Lung transplantation inevitably occur graft ischemia reperfusion injury (IRI), it seriously affects the overall prognosis of patients. How to prevent and treatafter lung transplantation is still a worldwide problem. At present, a large number of studies have found that inhaled nitric oxide (iNO) can treat hypoxemic respiratory failure and pulmonary hypertension after lung transplantation, and has great potential for the prevention and treatment of IRI. This review summarizes the mechanism of iNO in preventing and treating IRI after lung transplantation, discusses the optimal timing, concentration and duration of conducting iNO, further analyzing its efficacy, safety and side effects under specific application conditions to provide guidance for clinic.
