血管紧张素转换酶抑制剂(ACEI)和血管紧张素II受体拮抗剂(ARB)是高血压治疗的一线用药,在接受外科手术的高血压患者中有相当一部分服用此类药物。然而,在手术前是否应继续服用此类降压药存在较大争议,这可能归因于对继续服用药物的风险和获益的不确定性。文章就术前继续服用药物对患者术中低血压、术后器官损伤以及术后死亡率等围术期并发症的影响进行系统综述,以期为麻醉医生在临床实践中提供参考。
Angiotensin converting enzyme inhibitors (ACEI) and angiotensin II receptor blockers (ARB) are the first-line drugs for the treatment of hypertension, and are taken by a significant proportion of hypertensive patients undergoing elective surgery. However, there is a great controversy over whether such antihypertensive drugs should be continued before surgery, which may be attributed to the uncertainty about the risks and benefits of continuing such drugs. This article systematically reviews the effects of continuing these drugs on perioperative complications such as intraoperative hypotension, postoperative organ damage and postoperative mortality, in order to provide a reference for anesthesiologists in clinical practice.
