90% Effective Dose of Remimazolam for Anesthesia Induction with Painless Gastrointestinal Endoscopy in School-age Children

摘要：

目的：探索瑞马唑仑在学龄期儿童胃肠镜中的ED90，为瑞马唑仑的麻醉诱导应用提供更科学的临床剂量选择依据。方法：使用偏置硬币上下序贯试验测定瑞马唑仑的ED90 (使用R软件的Centered Isotonic Regression分析软件包计算)，诱导使用的镇痛药物是0.5 μg /kg瑞芬太尼，瑞马唑仑初始剂量为0.3mg/kg，相邻间隔升降剂量为0.02 mg/kg。结果：55名学龄期患儿纳入在本次研究中，瑞马唑仑的剂量范围是0.30-0.48mg/kg。使用Centered Isotonic Regression分析估计的ED90为0.448 mg/kg，未见瑞马唑仑严重不良反应。结论：在学龄期儿童胃肠镜检查的麻醉诱导中，配伍0.5μg /kg的瑞芬太尼的瑞马唑仑ED90为0.448 mg/kg (90% CI: 0.42-0.59 mg/kg)。

Abstract:

Objective To explore the ED90 of remimazolam in sedation for gastroscopy in school-age children, providing a more scientific clinical dose selection basis for the anesthesia induction application of remimazolam. Methods A total of 55 children were recruited into the study. The ED90 of remimazolam was measured using a biased-coin up-and-down sequential test (Estimated by the Centered Isotonic Regression analysis package of R software). The induced analgesic drug used was remifentanil 0.5 μg/kg, with an initial dose of remimazolam 0.3mg/kg and an adjacent interval dose 0.02mg/kg. Results 55 school-age children were included in this study, and the dose range of remimazolam was 0.30-0.48mg/kg. The estimated ED90 using Centered Isotonic Regression Analysis is 0.448 mg/kg. No serious adverse reactions were observed. Conclusions In the induction of anesthesia for gastrointestinal endoscopy in school-age children, the ED90 of remimazolam was 0.448 mg/kg (90% CI: 0.42-0.59 mg/kg) combined with 0.5μg/kg of remifentanil.
