星状神经节阻滞优化在改善术后睡眠障碍中的应用研究进展

星状神经节阻滞(Stellate ganglion block,SGB)随着超声引导技术的发展,其安全性、有效性和精准性大大提高,使其在疼痛、麻醉治疗学、睡眠医学等方面的使用越来越多,成为临床治疗疼痛性疾病,自主神经功能紊乱的疾病,失眠等常用的方法,既往研究发现星状神经节阻滞可以降低交感兴奋性,缓解睡眠障碍,改善脑血流,调节内分泌紊乱和自主神经紊乱,治疗失眠和创伤后应激障碍,抑制炎症反应等,因此其应用受到广泛的关注;术后睡眠障碍已经成为术后严重并发症中发生率最高的,星状神经节阻滞已被证明可以改善术后睡眠障碍;但是单纯使用星状神经节阻滞,仍然需要优化。目前暂未发现对星状神经节阻滞优化在术后睡眠障碍应用研究进展的综述;本综述将对星状神经节阻滞在引导技术,穿刺部位,穿刺时点,使用药物,以及与其他方式联合等方面的研究进行综述,为研究星状神经节阻滞的优化,以及其对术后睡眠障碍的治疗提供参考。

Abstract:

Stellate ganglion block (SGB) With the development of ultrasound guidance technology, its safety, effectiveness and accuracy have been greatly improved, making it more and more widely used in pain, anesthesia therapy, sleep medicine and other aspects, and becoming a clinical treatment for pain diseases and autonomic nervous disorders. Previous studies have found that SGB can reduce sympathetic excitability, relieve sleep disorders, improve cerebral blood flow, regulate endocrine disorders and autonomic nervous disorders, treat insomnia and post-traumatic stress disorder, and inhibit inflammatory response, so its application has attracted wide attention. Postoperative sleep disorders have become the highest incidence of serious postoperative complications, and SGB has been proved to improve postoperative sleep disorders. However, the use of SGB alone still needs to be optimized. At present, there is no review on the application of SGB optimization in postoperative sleep disorders. This review will review the research on SGB in guiding technique, puncture site, puncture time, use of drugs, and combination with other methods, so as to provide reference for the optimization of SGB and its treatment of postoperative sleep disorders.
