神经源性心肌顿抑(neurogenic stunned myocardium, NSM)是指心脏因急性神经事件产生的可逆性心肌功能障 碍。其发生率取决于相关神经系统疾病的触发因素及NSM的诊断标准。目前NSM的发病机制尚不明确,且极易与心肌原发 性损伤相混淆。文章系统介绍了NSM的流行病学、发病机制、临床表现、诊断标准、围手术期管理及预后等内容,以期对NSM 的临床决策能有所裨益,从而改善患者总体预后。
Neurogenic myocardial stunning (NSM) refers to reversible myocardial dysfunction caused by acute neurological events. The incidence of NSM depends on the trigger factors of related neurological diseases and its diagnostic criteria. Up to now, the pathogenesis of NSM is unclear and tends to be confused with primary myocardial injury. This review systematically introduces the epi⁃ demiology, pathogenesis, clinical manifestations, diagnostic criteria, perioperative management, and prognosis of NSM with an aim of improving the overall prognosis of patients.
