回顾性分析OSA儿童行药物诱导睡眠电影磁共振检查的麻醉管理

目的 探讨阻塞性睡眠呼吸暂停(Obstructive Sleep Apnea, OSA)儿童行药物诱导睡眠电影磁共振检查(Drug Induced Sleep Cine Magnetic Resonance Imaging, cine MRI)的麻醉用药和管理要点。方法 回顾性分析18例行cine MRI检查的OSA患儿资料,包括人口学状况、麻醉用药和监测、不同时点的生命体征和用药后发生的不良事件。结果 经cine MRI检查后18例OSA患儿中有11例(61.1%)发现阳性结果。右美托咪定诱导给药后,OSA患儿的心率明显减慢(P<0.05),且SpO2有所降低(P<0.05)。行cine MRI检查时有2例患儿因发生体动反应而追加氯胺酮,麻醉成功率为88.9%;有8例OSA患儿发生心动过缓,发生率为44.4%;所有患儿均未发生严重低氧(SpO2<90%)。阳性结果组患儿检查时心动过缓的发生率(63.6%)明显高于阴性结果组(P<0.05)。结论 右美托咪定是OSA儿童进行药物诱导睡眠电影磁共振检查时合适的麻醉药物,对呼吸抑制轻微,但需注意心动过缓,出现体动反应时可考虑氯胺酮作为联合用药。

Abstract:

Objective To explore the key points of anesthetic management for drug induced sleep cine magnetic resonance imaging (cine MRI) in children with obstructive sleep apnea (OSA). Methods Clinical data of eighteen cases of OSA children underwent cine MRI were analyzed retrospectively. Demographics, anesthetic drugs, vital sign profiles, as well as side effects were all included. Results After cine MRI examination, eleven of the eighteen OSA children (61.1%) were found positive results. By dexmedetomidine induction, OSA children occurred serious bradycardia(P<0.05), while SpO2 decreased significantly (P<0.05). During the cine MRI examination, there were two cases of OSA children with ketamine supplementation due to body movement, and the success rate of anesthesia was 88.9%. We found bradycardia in eight cases and the incidence was 44.4%. None of the children had severe hypoxia (SpO2<90%). The incidence of bradycardia in the positive results group (63.6%) was significantly higher than that of negative result group(P<0.05). Conclusion Dexmedetomidine is an efficacious drug used to induce sleep cine magnetic resonance imaging in OSA children with minimal respiratory depression. However, bradycardia is still noteworthy. Ketamine should be considered as a combination drug in case of body movement.
